The Tulip Pathway: Seamless Transition from Youth to Adult Care

The transition from children’s services to adult social care is often a challenging gap for vulnerable individuals. Because Tulip Community Care operates both Ofsted provisions (ages 11–18) and CQC provisions (ages 16–65), we are uniquely positioned to offer a seamless, non-disruptive pathway. For young people aged 16–18, we can carefully plan and execute transitions into adult-supported living without losing the familiar therapeutic team and clinical oversight they trust.

Who We Are?

Tulip Community Care is an independent, dual-registered care provider specialising in bespoke therapeutic placements and supported living accommodations. We have expanded our provisions to bridge the gap between youth and adult care, ensuring continuity, safety, and independence.

Our Ofsted Provisions (Ages 11–18): Specialised, trauma-informed residential care and therapeutic support for children and young people navigating complex emotional and mental health needs.

Our CQC Provisions (Ages 16–65): Comprehensive residential care and supported living packages tailored to young adults and adults, fostering independence while managing complex risks and mental health challenges.

What We Offer?

From our experience we also noted that most young people and young adults struggle to transition back into the community from hospital admission, placement breakdown from local authority placement which is where we get involved.​

Our service aims to work with the local authorities , commissioning groups and other independent stakeholders to set up bespoke placements for young people and young adults tailored to their needs whether the placement is closer to their community or if their needs indicates placement should be away from their community due to risks factors.

We offer a specialist therapeutic placement with an in-house multi-disciplinary with an extensive mental health experience. Our team consist of a Psychiatrist, Registered Manager, Psychologist, Social worker, Occupational Therapist, Clinical Nurse, Art Therapist, Speech and Language Therapist. The team with their expertise come together to offer top notch industry standard assessments whilst supporting our service users.

 

Tulip care will have the flexibility to set up bespoke packages recommendations by the professionals working with the person giving the stakeholders placement specification that meets the young person’s needs. The bespoke packages can involve sometimes sourcing other services from other agencies to ensure all placement specification are met.

 

Individual Multi-disciplinary team will assess the young person’s care and complete a holistic assessment to enhance care. Holistic practice draws on knowledge, theories, expertise, intuition and creativity to get the best outcome for our service users.

What We Do?

We provide bespoke community care packages with wrap around holistic care. We can on short notice setup bespoke one or two occupancy care homes, with experienced staff, MDT input, and aspire to be the leading provider in the region based on our unique model of care in setting up services.
